Part # for T11 Transformer on AD-FMCOMMS1-EBZ REV C

I recently knocked a transformer off of a board rendering it unusable.

I’d like to know what the manufacturer and part # for the T11 Transformer on the Analog Devices AD-FMCOMMS1-EBZ REV C is so that I can order the part and replace it.
